• Go to https://healthid.ndhm.gov.in/ and choose Generate ID.

• You will get an OTP at your registered mobile phone number.
• Enter the OTP and then your personal details before selecting your password and username.
• Login with your new password and username and give your address thereafter.
• Download the digital health ID card and you are done!

Through Driving License- 

• Go to https://healthid.ndhm.gov.in/ and choose Generate ID.
• Choose Generate ID via Driving License. You will get a pop-up where it mentions how you will get your enrolment number once you provide your information.
• Take this number to any nearby facility and get your health ID thereafter.

Through Mobile Number- 

• Go to https://healthid.ndhm.gov.in/ and choose Generate ID. 
• Choose Click Here if you avail the option for ID generation without any personal IDs.
• Enter the mobile number, get the OTP and enter the same on the page.
• Key in your personal details and select your password and username.
• Login to the account with your new details and give your address.
• Download the digital health ID card likewise.

Documents Required for Digital Health ID Card

You will basically require either your mobile phone number or your Aadhar number or your driving license.
