1. Age Proof

You will be required to show evidence of age for every individual who will be insured by the medical insurance policy you choose. You may submit any of the following legal papers.

    • Aadhaar Card
    • Secondary or Higher Secondary Exam Mark Sheet
    • Passport
    • Driver’s License
    • Voter’s ID Card
    • PAN Card

2. Proof of Identity

One of the following types of identification will be required of you.

    • Aadhaar Card
    • Passport
    • Voter’s ID Card
    • Driver’s License
    • PAN Card

3. Proof of Address

You will be asked to provide permanent evidence of address. For the same, you may provide any of the following legal papers.

    • Aadhaar Card
    • Electricity Bill
    • Voter’s ID Card
    • Telephone Bill
    • Driver’s License
    • Passport
    • Ration Card

4. Passport-size photographs

5. If you are required to undergo a medical test, it is also necessary for you to submit the results of the tests.

6. Aside from these papers, insurance firms may request additional documents or any other information based on company regulations and the inspection of your application.
